REPORTING OF DEATHS OF FOREIGNERS AND NON-INDIANS IN INDIA

1.  Name of the deceased.

2.  Nationality. 

3.  Profession. 

4.  Date and Place of birth. 

5.  Date and Place of death. 

6.  Cause of death. 

7.    Particulars of passport held by deceased  

8.    List of personal belongings and their disposal
 

9.  Relatives/friends, if any left behind in India. If so,
whether they are aware of the event.

 10.  Address of any relatives in the country of the
 deceased, if any, known to the State
 Government.

 11. Details of movable/immovable property, if any left
  behind in India.

12. Death certificate, in prescribed pro-forma, duly
      attested by the authorized officers of the concerned 
      State Government.

 

13.  Post-mortem Report in prescribed form duly attested
