Procedure for New Registration

Application  forms are  available at C.P.R.C. Kapurthala (Click here for the forms ). Three sets of these forms are required to be submitted alongwith the following documents :-

a)   4 copies of photograph.
b)   Photocopy of the Passport.
c)   Photocopy of the  Visa.
d)   Photocopy of passport showing date of arrival.
e)   Residential proof of local person (Ration Card, Voter
      card, Licence etc)  who had taken surety.
